We’re on the precipice of an incredible opportunity to reimagine how we deliver care to patients in their homes – but it will require coordination across payors, providers, and the myriad of home care delivery assets and technology solutions. In this session, Tomorrow Health’s Vijay Kedar gathered insights from Patrick Conway and Dr. Jaewon Ryu about how integrated systems are approaching the home as a setting; the importance of partnerships and collaboration to link home-based care programs with acute and post-acute care; and the transition to driving value in home-based care through organizations taking on more risk.

The future of the hospital
In the wake of COVID and amidst a growing financial crunch, hospital systems are facing challenges that are likely here to stay. Between nursing shortages, inflation, and an aging population with increasing chronic disease needs, hospital systems are looking at consolidation, innovative partnerships, and applications of generative AI to meet these challenges head-on. Dr. Toby Cosgrove and Julie Yoo shared their thoughts on the migration of care to the home – and why the industry needs more specialized companies that have expertise in home-based care to supplement that care delivery.

How policy will impact care delivery in 2033
2023 was a tumultuous year for healthcare policy, as the Public Health Emergency ended and the federal government kicked off Medicaid redetermination – impacting millions of people and their access to healthcare. Regulatory and policy efforts aimed at fostering value-based payment alignment in Medicaid will be critical in the evolution of home-based care as a mainstream care strategy. Eric Hargan, Aditi Malick, and Stuart Portman offered their perspectives on the nuances of Medicaid, and what we can expect to see from a policy perspective in the next decade.

What home-based care looks like in action
Dr. Tad Funahashi and Stephanie Gonzalez of Kaiser Permanente presented a case study of a novel home-based cardiac rehab program developed by Kaiser Permanente’s Southern California Innovation team in collaboration with Cardiology specialists. By shifting cardiac rehab to the home, the Kaiser team has seen over 16,000 patients complete the program, and has achieved fewer hospital admissions and lower secondary mortality and morbidity – demonstrating what a successful at-home program like this can actually look like.

Today’s hospital and tomorrow’s home-based care
When comparing and contrasting different home-based care programs across Blues plans, integrated health networks, and risk-bearing providers, it was clear that identifying the right financial model and payment incentives will help support value-based care programs and reduce total cost of care outcomes. Panelists across the spectrum of health plans expressed they are living in “two different worlds” in terms of driving progress of home-based care programs for their populations under value arrangements, while still needing to provide and manage care for fee-for-service populations. The consensus? The technology layer is critical – and as the technology improves, our ability to serve patients in their homes will grow exponentially.
